Laparoscopic Procedures For Pelvic Pain

Dear Dr. Ransom,

I’m 35 years old and have had two previous laparoscopic procedures for pelvic pain. I am looking for long-term relief. What can I do?

Many women suffer from what is known as “chronic pelvic pain”. This is a condition defined as pain of greater than six months’ duration. There are many causes of chronic pain.

Gynecologic causes include:
  • Endometriosis

  • Adenomyosis (glands of the lining of the uterus which have grown into the muscle of the uterus)

  • Chronic pelvic inflammatory disease (caused by prior sexually transmitted infections)

  • Adhesions (internal scarring as a result of previous surgical procedures).

Pain which arises from nongynecologic causes may include:

It is critical to the successful treatment of chronic pelvic pain to tailor the management to the cause of pain. It is also important to rememeber that pain is a matter of perception, and that perception may be altered in women who have been the victim of physical or sexual abuse.


For the woman with severe and recurrent symptoms of chronic pelvic pain as a result of endometriosis and who has also completed her childbearing, a total abdominal hysterectomy and removal of the ovaries is appropriate.
